Thank you for taking the first step towards becoming a Virta patient!

Our team, Virta Support, looks forward to guiding you through our process. For reference, we have put together an overview of required steps below.

*In each phase (i.e. Application and Screening), each step is required, but they do not have to be done in the order listed above. For example, in the Screening phase, you can have your tests done before your Virta Physician consultation.**Be sure to use the same email address throughout the application process.

Phase 1: Application

Basic Information and Health History Forms

  • Purpose: To give our team a snapshot of your health history, so (i) our physician team can make a preliminary decision on whether Virta makes sense for you and (ii) your informational conversation with a Virta Specialist can be as personalized as possible.
  • What you need to do: Complete both forms as completely as you can. You will be asked about your health, your medications, your immediate family’s health, and for your primary care physician’s contact information.
  • Time Required to Complete: 20 minutes

Release of Information (ROI) Authorization

  • Purpose: To give our Physician team the ability to communicate with your primary care physician.
  • What you need to do: Complete both forms as completely as you can. You will be asked about your health, your medications, your immediate family’s health, and for your primary care physician’s contact information.
  • Time Required to Complete: 20 minutes

Intake Call

  • Purpose: To speak with a member of our Virta Specialist team and gain an understanding of Virta’s approach and what being a Virta patient will be like for you.
  • What you need to do: Schedule a time for your conversation through Calendly. When you reserve a time, be sure to use the same email address as the one you provided on the basic information form. At the time of your appointment, your Virta Specialist will call the number you provided.
  • Time Required to Complete: 20-45 minutes

Phase 2: Screening

Virta Physician Consultation

  • Purpose: To establish care and get acquainted with the Virta physician who will supervise your time as a Virta patient.
  • What you need to do: Schedule an initial consult through Calendly. This appointment will be completed through a telehealth platform, Doxy.me. Once you reserve a time, you will receive a URL and additional directions on how to ‘arrive’ to your appointment on a smartphone, tablet, or desktop.

  • Time Required to Complete: 45 minutes

Tests for Medical Clearance

  • Purpose: To receive a final decision from your Virta physician on your application to start as a patient.
  • What you need to do: Download your lab orders from the email titled ‘Next Step for Virta Enrollment: Labs & EKG’. If you are receiving Virta through your employer, a specified healthcare provider will be listed in your lab email. In all other cases, contact your local physician to get the labs and EKG listed done. At the time of your testing, stress to your healthcare provider that you want the results faxed to Virta (1-888-974-1469).
  • Time Required to Complete: On average, 7 days for results to arrive at Virta. This step varies based on your local physician’s availability and time it takes for labs to be processed.

Approval and Getting Started

Once your consultation is complete and test results are in, you will be ready to start as a Virta patient! To make sure you are set up for success, we will be giving you a starter kit, access to your Virta Account, and introducing you to your new Health Coach!What you need to do to:

  • Receive Your Starter Kit: You will receive an email titled ‘Congratulations! You’ve been accepted into the Virta Clinic!’. In your reply, share the address we should use to ship your starter kit. You or a representative will need to be able to sign for your kit when it arrives.
  • Create Your Virta Account: In an email titled ‘XXX’, click on the provided link. You will be taken to a page where you can create a password for your account. This link expires within 24 hours, but you can reach out to support@virtahealth.com if you miss this time window.
  • Meet Your Virta Health Coach: Through an email titled ‘Meet your Health Coach and get started on Virta’, schedule a time to meet your Health Coach through their Calendly link.
  • Time Required to Complete: 30-50 minutes
  • Receive Your Starter Kit: 2 minutes
  • Create Your Virta Account: 2 minutes
  • Meet Your Virta Health Coach: 30 - 50 minutes
